My name is Krista Elliott and I am the Registered Nurse for Brywood Elementary. To ensure that the medications you send with your student to Outdoor Ed camp is administered to your child, you must follow the specific guidelines as outlined on the “Instructions for Completing Medication Authorization Form” included in your packet. The following are important reminders when completing the authorization form.
Important Information to Remember Regarding Medications and Camp
1. Please remember to follow all the instructions for sending medications to camp precisely.
2. Please send all medications in a large zip lock bag. Bring it to the health office no later than October 31st.
3. All medications MUST BE PRESCRIBED BY A LICENSED CALIFORNIA PHYSICIAN.
4. Please note that all NON PRESCRIPTION MEDICATIONS (over the counter medications, ie Tylenol, Motrin, Vitamins etc.) must ALSO be prescribed by a physician in order to be given at camp. If a doctor did not prescribe a medication, it will not be given, no exceptions.
5. Please check to make sure that the medication is Not expired prior to sending to school. Expired medications will not be given.
6. Please remember that all Medications that go to camp MUST COME FROM YOUR HOME SUPPLY. The student’s medications that are at school will not be used for camp. They will remain at Brywood.
7. All medications must be in original pharmacy container with the correct and appropriate dosage for the student’s size and age.
